2011 ADP 7699—In the matter of the adoption of Tayler Lee Turk.
Donald Sharp Morgan, being first duly sworn, deposes and says that he is the petitioner in the above entitled proceedings for the adoption of the minor child of Jason Calhoun and Angela C. Morgan, parents, that Jason Calhoun, aforementioned, does not reside within the State of Ohio, his last place of residence being unknown.
It is alleged that Jason Calhoun has failed without justifiable cause to provide more than the de minimis contact with the minor for a period of at least one year immediately preceding either the filing of the adoption petition or placement of the minor in the home of the petitioner. That other than above set forth, the residence of said parent is unknown and cannot with reasonable diligence be ascertained; that this case is mentioned in Section 3107.11 of the Revised Code of Ohio, requiring compliance with the Ohio Rules of Civil Procedure as to notice.
Said cause is ordered set for hearing on the 15th day of March, 2012, at 9:00 a.m., or as soon thereafter as the Court may hear the same.
