914580—Treasurer of Cuyahoga County, Ohio vs. Unknown Heirs of Shelva C. Blazer aka Shelva Charlene Blazer, et al.
Unknown heirs, devisees, legatees, assignees, executors, administrators and legal representatives of Shelva C. Blazer aka Shelva Charlene Blazer, the address of each being unknown, will take notice that on September 1, 2021, the undersigned, State of Ohio Medicaid Collection Division, filed its answer and cross-claim in the Court of Common Pleas, 1200 Ontario Street, Cleveland, Ohio 44113, of Cuyahoga County, Ohio, alleging that Shelva C. Blazer was a recipient of funds from the State of Ohio Medicaid Division for the period beginning October 1, 2011 and continuing monthly through June of 2014; that the total of all funds paid on behalf of Shelva C. Blazer for the above-referenced period totaled $39,777.29; that there is truly due and owing State of Ohio Medicaid Collection Division from the excess proceeds the sum of $39,777.29, and this answering Defendant is due, pursuant to ORC §5162.21, et seq, from the within foreclosure said amounts as stated above; and that none of said $39,777.29 due and owing has been paid to date.
The State of Ohio Medicaid Collection Division prays that its claim remain in statutory priority for full or pro rata payment from the remaining proceeds, that this Court order the payment from the excess funds from the within foreclosure, up to a maximum of $39,777.29, to the State of Ohio Medicaid Collection Division, for Court costs, and for such further equitable relief as this Court may deem just and proper.
The defendants named above are required to answer on or before the 11th day of November, 2021.
